  • Included in this package are my written recommendations. It is the parent(s)/client(s) responsibility to schedule future care. Once an appointment is made, I am happy to have a telephone consultation with their pediatrician or mental health provider. Please sign a Release of Information (ROI) with their provider and email a copy.
